Step 2: Save the completed application to your hard-drive, then complete the form below and attach the application to submit.
You will receive notification via email or phone as to whether your application is accepted.
Step 3: After your application is accepted, you will be contacted to conduct a Business Assessment and review program guidelines.
Step 4: After the completion of your Business Assessment, you will work with your program manager to create a customized Technical Assistance Plan. This plan is developed with participants' input and will outline the areas of support agreed upon between you and your program manager.
Applicants who do not meet program criteria will be referred to other resources.
